A face to face assessment with the senior is the basis for the senior's residency care plan. This is a state requirememt. Re-assessments are conducted by the community's care team at 30 days of residency and then every 90 days.
Care plans are adjusted when necessary. For example, when a senior experiences a change in condition before the required
re-assessment.
